Licensed in Utah, Texas, New Mexico, and Illinois, Corby brings a broad professional foundation and a strong commitment to ethical, compassionate care. Therapy here is centered on substance abuse, family conflict, relationship challenges, and self-esteem concerns, with a steady, compassionate presence in each session.
The goal is simple: to help you reconnect with who you really are and move forward with more clarity, purpose, and authenticity. Requesting Your Health Care Records
You have the right to request a copy of your mental health records. To do so, submit a written request to your treating clinician or directly to the practice. Requests may be submitted by email, through the secure client portal, or in writing. Records will be provided in accordance with Texas law and applicable privacy regulations. If you have questions about accessing your records, please contact our office for assistance.

Filing a Consumer Complaint
If you wish to file a consumer complaint regarding mental health services, you may do so with the Texas Office of the Attorney General:

Good Faith Estimate Notice
You have the right to receive a Good Faith Estimate of expected charges before services begin if you are uninsured or choose not to use your health insurance. A written estimate will be provided upon request or at the time of scheduling. This estimate is not a contract, and actual charges may vary. If your bill exceeds the estimate by $400 or more, you may dispute the charge through the federal dispute resolution process.
Medicare Policy Disclosure
This practice does not participate in the Medicare program. If Medicare has been opted out of, an official Medicare opt-out affidavit has been filed, and you will be responsible for private-pay fees. You may choose to seek reimbursement from Medicare, as permitted by law.
